The City is currently soliciting competitive bids for the following:

Mowing Various Private Lots -- Bid Opening Date February 16, 2012, 2 p.m

Refurbish 2003 Tymco Street Sweeper Model 600 -- Bid Opening Date February 21, 2012, 2 p.m.

2012 Leak Detection Services -- Bid Opening Date February 21, 2012, 2 p.m.

Water and Sewer Supplies for 2012 -- Bid Opening Date February 21, 2012, 2 p.m.

Request for Qualifications (RFQ) -- Clean Ohio Revitalization Fund for former Hooven & Allison Cordage Property ... RFQs are due no later than 3 p.m. on Friday, March 2, 2012.

HVAC Maintenance Agreements for City Facilities -- Bid Opening Date February 28, 2012, 2 p.m.

Crack Sealing of Asphalt Bituminous Pavement -- Bid Opening Date February 28, 2012, 2 p.m.

Wide Area Network Services  -- Bid Opening Date March 9, 2012, 2 p.m.

For your convenience, the City of Xenia is pleased to offer you the following fill-in forms, which may be required for bid submittal:

Delinquent Tax Affidavit

Non-Collusion Affidavit

Campaign Contribution Certificate

Bid Bond

Corporate Certification

The forms were created in Adobe Acrobat format, which is also known as PDF format (PDF = Portable Document File). To view, fill out, and/or print a form, you will need an Adobe Acrobat Reader installed on your computer. The Adobe Acrobat Reader is a FREE program, which can be downloaded from the Adobe website. Click on the "Get Adobe Reader" icon below to download the program.

 

Please check your bid specifications to verify which forms are required for the particular item/project you are bidding on, as all forms listed above may not be required. Further, all forms that are required may not be available as a fill-in form.

IMPORTANT NOTE: If you choose to fill out a form when it appears on your computer screen in the Adobe Acrobat Reader program, any data you enter onto a form CANNOT be "saved"--it can only be printed. When you close a form, the data you have entered will be deleted. Therefore, if you choose to fill out a form on your computer, be sure to check for accuracy and print a copy upon completion. Further, be sure to sign the form(s) where applicable.
